This form is used to collect data relating to clients of Aziaccountant Chartered Accountants for the purposes of verifying their identity and to comply with anti-money laundering legislation. Please use this form to submit the information and documents requested, as this method of data collection is intended to protect the security of your personal information, as required by the General Data Protection Regulation. Do not send such information or documents by email, as we are unable to guarantee the security of emails in transit before they reach our systems

Proof of Identity

- a passport or photo driving license; AND

- a utility bill or similar proof of address dated within past 3 months.

Please note:

- you must provide TWO documents for each shareholder holding 25% or more of the company's share capital.. A driving licence can be used either as a photo ID OR proof of address. It cannot be used for both

- a mobile telephone bill is NOT suitable as proof of address.

Please ensure all images are clear. If there are issues with your documentation, you may be asked by the Compliance Team to upload them again.
